TERMS & CONDITIONS

Please read the terms and conditions below carefully on which Facial Aesthetics by Dr Hogg provides services to its patients.  If you have any questions, please contact us prior to booking your appointment with Facial Aesthetics by Dr Hogg.

BOOKING & CANCELLATION POLICY

Appointments can be made via text, phone, email, our website or social media channels.  Your appointment will be confirmed by email and/or text and an appointment reminder email and/or text is sent 72 hours before your appointment.  If you do not receive these confirmations and/or reminders, please let us know immediately.  We cannot be held responsible if these reminders are not received and you do not attend your scheduled appointment.

Facial Aesthetics by Dr Hogg requires at least 48 hours notice to cancel or reschedule your appointment.  An appointment can be cancelled or rescheduled by texting, emailing, telephoning or contacts us via our social media channels.  Booking cancellation will be confirmed by email and/or text.  Failure to provide at least 48 hours notice, will result in the forefeit of your booking fee.  If you are unwell on the day of your appointment, please contact us so that we can speak with you over the phone.

Appointments may occasionally have to be cancelled or rescheduled by the clinic, including at short notice, should any emergencies or equipment failures occur.  Facial Aesthetics by Dr Hogg will not be liable for consequential loss due to the cancelling or rescheduling a patients appointment.

PAYMENT TERMS

All appointments booked require payment of a non-refundable booking fee in order for the appointment time to be reserved.  This booking fee is redeemable against any treatment.  The booking payment can be paid either in cash or via BACS transfer and receipt of this will be confirmed by text and/or email.  The booking fee is usually £50, however, this will be confirmed at the time of booking.

The cost of all treatments must be paid, in full, in advance or at the time of treatment.  Accepted payments methods include BACS, debit or credit card payment or cash.  Cheques are not accepted.

LATE ARRIVALS

If you arrive late more than 20 minutes behind schedule for your appointment, your treatment may be cancelled and/or re-scheduled.  In such event, your booking fee may be forfeited.

Facial Aesthetics by Dr Hogg aims to run the clinic efficiently, however, there may be times when the clinic runs behind schedule.  If the clinic is running more than 20 minutes behind your scheduled appointment time, we would be happy to reschedule it for you.  Your booking deposit would be transferred to your new appointment.

COMFORT & SAFETY

Facial Aesthetics by Dr Hogg operates a strict no smoking policy within the building.  Mobile phones must be switched off or put on silent whilst in the clinic.  Please do not undertake audio or video recording whilst in the clinic or during your treatment. 

CHILDREN

Please do not bring children to your appointment.  Children cannot be brought into the clinic room and cannot be left unaccompanied on the premises.

REFUSAL OF TREATMENTS

Facial Aesthetics by Dr Hogg reserves the right to refuse treatment where our clinicians deem it inappropriate for any reason and their decision is final in this regard.  We do not provide treatment to anyone under the age of 18 years.

ACTS OF GOD

Facial Aesthetics by Dr Hogg does not issue refunds for Acts of God.  An Act of God is defined as an event outside of human control such as sudden floods, earthquakes or other natural disasters, for which no one can be held responsible.  This also includes weather-related issues such as snow, ice, floods etc.  If Facial Aesthetics by Dr Hogg cancels a clinic due to an Act of God, then it is not liable for any loss resulting to the customer as a result of this cancelled clinic.

COMPLAINTS

Facial Aesthetics by Dr Hogg takes pride in the quality of its service.  If you are unhappy with your treatment or service, complaints can be made in writing by emailing info@drhoggclinics.co.uk.  Complaints will be acknowledged by return email within one week of receipt and a response will be made within an appropriate time, usually 20 working days.  If it will take longer than this time for a response to be made, you will receive written notification of this.

REFUND POLICY

The result of any procedure or treatment cannot be guaranteed.  No refunds can be given if the results achieved fail to meet your expectations.  Due to the nature of medical treatments, results may vary from person to person.

Facial Aesthetics by Dr Hogg will not be liable in contract, tort or otherwise for any economic loss (including without limitation, loss of profit), or any other special, indirect or consequential loss or damage arising out of, or in connection with, its provision of any goods and/or services to the patient.

PRIVACY POLICY

Facial Aesthetics by Dr Hogg is committed to protecting our customer’s privacy.  We treat personal data in accordance with our obligations by the Information Commissioner’s Office.

The information that we collect about you may be used for different reasons:

1. Enquiries – We may record your details in order to track and follow-up on enquiries, in addition to scheduling appointments. This includes information such as your name, e-mail address or phone number. This is so that we can supply you with the information that you have requested about our professional services, on the basis that it is necessary for our legitimate interests in promoting and in order to provide information and a quotation for our services.  Failing to provide us with this information for marketing purposes will have no impact upon the provision of your treatment.
2. Medical Records – Information must be collected as part of your registration with the clinic. This includes information such as your name, address, date of birth, e-mail address, phone number, next of kin and medical history. This is so that we can perform the contract that we have with you, for internal record keeping, billing and accounting, and to respond to any queries, complaints or requests for further information. The basis on which we do so is that it is necessary for our performance of the contract we have with you or is necessary for our legitimate interests in managing our business and improving our professional services, and to comply with our regulatory obligations.  Failing to provide us with this information will result in us being unable to provide treatment.  Medical records are stored electronically in Clever Clinic; the separate terms of use are available at: https://cleverclinic.co.uk/terms

All our treatments require us to hold information regarding our patients personal details including medical information.  The information remains confidential and is stored as per the Information Commissioner’s Office.  No information will be sold to third parties.  As part of your treatment you will be required to complete a medical questionnaire and report any changes at future appointments.  It is your responsibility to disclose all information required and Facial Aesthetics by Dr Hogg will not be held responsible for any undisclosed information.

You may request details of the personal information which we hold about you under the General Data Protection Regulation (GDPR).  If you would like a copy of the information held on you please write to us at info@drhoggclinics.co.uk.  We aim to supply information within 1 month of receipt of your request and all requests will be subject to the appropriate identification checks.

If you believe that any information we are holding on you is incorrect or incomplete, please write to or email us as soon as possible at the above address.

We do not store credit card details nor do we share customer details with any third parties.

In some circumstances, you have the right to be forgotten and we will erase all data held about you. Medical records are exempt from erasure. Requests for erasure should be made in writing to the address above.
